The reemergence of the fly could suggest that Walt's cancer came back.

I do not believe that he is "out," as he says though. Perhaps he is relocating so Skyler and the kids can live in peace? If you recall, the season opener showed Walt celebrating his birthday by himself in Vermont I believe. That was presumably the future. So many possibilities though. I've been speculating non-stop, and it will be hard to wait a whole year to see what happens.

Vince Gilligan said in an interview after The Fly episode that the fly was Walt's conscience. Certainly could work that way here as well. I think the cancer return is clear enough from seeing Walt in the MRI and then him not telling anyone about it for at least three months, then there was all that coughing in the season opener scene that you mentioned.

Cranston made a comment in the Rolling Stone article about what's going on in that scene that you may or may not want to read depending on how un-spoiled you want to be.

So many great open questions for next year ...

what the hell is up with Todd?
will they really let Walter out even if he means it?
what will Hank do?

and many, many more ...
